Endothelial dysfunction is a pathological state caused by an imbalance between vasoconstriction and vasodilatation. Besides smoking, aging, and hypertension, recent studies have shown adiposity, dyslipidemia, and chronic inflammation as factors increasing endothelial dysfunction. Endothelial dysfunction causes an increase in the levels of chemokines, leukocyte adherence, cell permeability, low-density lipoprotein oxidation, platelet activation and a decrease in nitric oxide levels.

The current research digest highlights some of the recent therapeutic modalities used for improving endothelial function.

In the above study, circuit resistance training was found to be an effective modality for improving endothelial function in obese women. The study comprised 48 obese women, 24 of which were randomly selected for a 12 weeks resistance circuit training program. The program was performed 3 days a week and it included 6 different exercises: arm curl, leg extension, seated row, leg curl, triceps extension, and leg press. In addition, Jamar handgrip electronic dynamometer was used to assess maximal handgrip strength of the dominant hand. Serum samples of the patients were analyzed for endothelin-1, intercellular adhesion molecule-1 (ICAM-1), and vascular cell adhesion molecule-1 using ELISA. On evaluation, it was seen that compared to the baseline, resistance training significantly reduced serum levels of endothelin-1 (2.28 ± 0.7 vs. 1.98 ± 1.1 pg/ml; P = 0.019; d = 0.67) and ICAM-1 (290 ± 69 vs. 255 ± 76 ng/ml; P = 0.004; d = 0.92) in the experimental group. There were no significant changes found in the control group.

In the current study, the efficacy of L-arginine (L-Arg) and Vitamin C supplementation was checked as a potentially therapy for modulating serum homocysteine (Hcy), tumor necrosis factor alpha (TNF-α), oxidative stress, and insulin resistance in obese Wistar rats. Six-week-old female and male Wistar rats were divided into five groups of 12 rats each. Each group was treated for 6 weeks, Group 1: standard diet; Group 2: high fat diet (HFD); Group 3: HFD supplemented with L-Arg (20 g/kg diet); Group 4: HFD supplemented with L-Arg (20 g/kg diet) plus Vitamin C (100 mg/kg diet); Group 5: HFD supplemented with Vitamin C (100 mg/kg diet). In the 6 th week, an increase in TNF-α, homeostatic model assessment-insulin resistance, total cholesterol, low-density lipoprotein (LDL) cholesterol, triglyceride, Hcy and a subsequent decrease in total antioxidant status (TAS) and high-density lipoprotein cholesterol levels were observed in HFD rats. On supplementation with L-Arg, there was a significant reduction in the levels of TAS, Hcy, and TNF-α levels. On the other hand, Vitamin C remarkably attenuated TAS decrease and lowered LDL cholesterol levels. Moreover, the combination of L-Arg and Vitamin C enhanced L-Arg effect on TAS and protected against TNF-α increase. Western blot analysis showed that L-Arg supplementation in HFD reduced the level of protein carbonyls. These findings thus indicate that L-Arg and/or Vitamin C are anti-atherogenic since they modulate the biomarkers of HFD-induced endothelial dysfunction.

In this study, the role of Vitamin B6 in endothelial function was investigated. Apoe mice were divided into two groups. Groups 1 and 2 were on HFD and HFD with Vitamin B6 (50 mg/kg/day) for 8 weeks. The parameters determined were endothelial dysfunction, insulin resistance, and hepatic lipid contents. It was noticed on the 8 th week that mice on HFD with Vitamin B6 showed an increase in acetylcholine-induced endothelium-dependent relaxation and a decrease in random blood glucose level and hepatic lipid accumulation. Vitamin B6 also improved the insulin sensitivity, normalized the histopathology of the liver with no change in liver enzyme levels. Thus, Vitamin B6 supplementation should be considered for prevented endothelial dysfunction in adiposity.
